Inflammation of the tendon plate on the sole of the foot at the level of the tendon insertion on the heel bone can lead to pain, which occurs particularly with pressure and strain. Plantar fasciitis is usually self-limiting but can be long-lasting. Imaging techniques are used in cases of suspected diagnosis or for differential diagnostic evaluation.<\/strong><\/p>\n<p> <!--more--> <\/p>\n<p>Fasciitis plantaris is a relatively common inflammatory reaction of the plantar fascia that is painful and limits mobility and weight-bearing [4]. Short-term unusual stress [2] should also be recorded as a cause of the plantar inflammatory reaction. MRI studies have also demonstrated the influence of adjacent muscular changes on the development of plantar fasciitis [3,5,6]. These are listed in <span style=\"font-family:franklin gothic demi\">overview&nbsp;2<\/span>. The symptoms are similar to those of plantar calcaneal spurs: stabbing pain when stepping on, dull, irregular pain in the heel area even without weight-bearing, typically also start-up pain especially early in the morning, tenderness at the tendon insertion [9].<\/p>\n<p>&nbsp;<\/p>\n<p><img decoding=\"async\" class=\"size-full wp-image-16661 lazyload\" alt=\"\" data-src=\"https:\/\/medizinonline.com\/wp-content\/uploads\/2021\/07\/ubersicht2_hp6_s32.png\" style=\"--smush-placeholder-width: 907px; --smush-placeholder-aspect-ratio: 907\/332;height:146px; width:400px\" width=\"907\" height=\"332\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\"><\/p>\n<p>&nbsp;<\/p>\n<p>Therapeutically, a wide range of conservative measures is initially available<span style=\"font-family:franklin gothic demi\"> (overview&nbsp;3)<\/span>.<\/p>\n<p>Surgical treatment should be performed only after conservative treatment has been exhausted.<\/p>\n<p>&nbsp;<\/p>\n<p><img decoding=\"async\" class=\"size-full wp-image-16662 lazyload\" alt=\"\" data-src=\"https:\/\/medizinonline.com\/wp-content\/uploads\/2021\/07\/ubersicht3_hp6_s32.png\" style=\"--smush-placeholder-width: 922px; --smush-placeholder-aspect-ratio: 922\/533;height:231px; width:400px\" width=\"922\" height=\"533\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\"><\/p>\n<p>&nbsp;<\/p>\n<p>Radiographs in lateral projection cannot capture the inflammatory response of the plantar fascia. If a plantar heel spur is visible, people tend to think that it is the cause of the symptoms. However, it is important to remember that fasciitis and spur can occur simultaneously. If no heel spur is recognizable, fasciitis plantaris may be suspected as the trigger of the load-dependent complaints, often also in combination with rarefying fibroostitis calcanei. Inflammation leads to thickening and reduced echogenicity [7]. Surrounding fluid collections are often detectable when inflammation is pronounced.<\/p>\n<p><em>Computed tomographic examinations<\/em> can visualize associated bony changes, such as the plantar calcaneal spur and calcifications of the plantar fascia, very well <span style=\"font-family:franklin gothic demi\">(Fig.&nbsp;3A); the<\/span> extent of inflammation of the aponeurosis itself is poorly assessable.<\/p>\n<p><em>Magnetic resonance imaging<\/em> already shows fasciitis plantaris <span style=\"font-family:franklin gothic demi\">(Fig.&nbsp;3B) <\/span>in native sequences, especially fat-suppressed sequences [8] <span style=\"font-family:franklin gothic demi\">. <\/span> The detection and assessment of adjacent inflammatory soft tissue and bone changes is even better with contrast support.<\/p>\n<h2 id=\"\">&nbsp;<\/h2>\n<h2 id=\"-2\"><img decoding=\"async\" class=\"size-full wp-image-16665 lazyload\" alt=\"\" data-src=\"https:\/\/medizinonline.com\/wp-content\/uploads\/2021\/07\/abb3_hp6_s33.png\" style=\"--smush-placeholder-width: 1100px; --smush-placeholder-aspect-ratio: 1100\/504;height:275px; width:600px\" width=\"1100\" height=\"504\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\"><\/h2>\n<h2 id=\"-3\">&nbsp;<\/h2>\n<h2 id=\"case-studies\">Case studies<\/h2>\n<p>In <strong>case report 1<\/strong>, the lateral radiograph of the right heel demonstrates complex enthesiopathy in a 70-year-old patient with load-dependent pain in the plantar and dorsal heel area and in the tarsal area with osteoarthritis. Plantar and dorsal heel spurs as well as calcifications of the plantar fascia and in the attachment of the Achilles tendon are indications of chronic inflammatory degenerative changes.<\/p>\n<p>In <strong>case 2<\/strong>, the high soft tissue contrast of MRI and intravenous contrast injection (Gd-DTPA) can reliably demonstrate florid inflammation of the plantar fascia <span style=\"font-family:franklin gothic demi\">(Fig.&nbsp;2) <\/span>. The 67-year-old patient had load-dependent plantar pain that limited her mobility.<\/p>\n<p><strong>Case report 3<\/strong> demonstrates a 48-year-old man (postoperative condition after calcaneus fracture) with hypertrophic plantar calcaneal spur in addition to significant secondary arthrotic changes of the ankle and tarsal on CT <span style=\"font-family:franklin gothic demi\">(Fig.&nbsp;3A)<\/span>.
