Il est g\u00e9n\u00e9ralement admis que la photo-d\u00e9gradation chronique de la peau par les rayons ultraviolets est le facteur le plus important dans l&#8217;\u00e9tiologie des carcinomes \u00e9pidermo\u00efdes.<\/strong><\/p>\n<p> <!--more--> <\/p>\n<p>Par le biais d&#8217;effets directs et indirects, l&#8217;exposition aux UV endommage l&#8217;ADN des k\u00e9ratinocytes \u00e9pidermiques. En cas d&#8217;exposition excessive et avec l&#8217;\u00e2ge, les m\u00e9canismes de r\u00e9paration peuvent \u00eatre d\u00e9pass\u00e9s et des mutations canc\u00e9rig\u00e8nes peuvent alors appara\u00eetre. Environ 90 % des carcinomes \u00e9pidermo\u00efdes apparaissent sur des zones expos\u00e9es au soleil, comme le capillitium non poilu, le front, l&#8217;ar\u00eate du nez, les h\u00e9lices des oreilles, les avant-bras et le dos des mains [1]. En Allemagne, le carcinome \u00e9pidermo\u00efde est class\u00e9 comme maladie professionnelle en raison de l&#8217;exposition professionnelle aux UV chez les travailleurs ayant une part importante d&#8217;activit\u00e9s en plein air, comme les ouvriers du b\u00e2timent, les travailleurs agricoles ou horticoles [2]. Une analyse secondaire a montr\u00e9 que l&#8217;odds ratio (OR) regroup\u00e9 pour l&#8217;association entre le travail en plein air et le risque de d\u00e9velopper un carcinome \u00e9pidermo\u00efde est de 1,77 (IC 95% : 1,40-2,22) [3,4].
