Les th\u00e9rapies syst\u00e9miques peuvent certes prolonger la survie, mais elles n&#8217;atteignent pas les cellules tumorales dans le cerveau. Une cible th\u00e9rapeutique potentielle pourrait \u00eatre la voie de signalisation PI3K\/Akt\/mTOR.<\/strong><\/p>\n<p> <!--more--> <\/p>\n<p>Les options de traitement des m\u00e9tastases c\u00e9r\u00e9brales du m\u00e9lanome malin sont actuellement tr\u00e8s limit\u00e9es. L&#8217;une des consid\u00e9rations \u00e9tait donc l&#8217;inhibition de la voie de signalisation PI3K\/Akt\/mTOR (PAM), \u00e9tant donn\u00e9 qu&#8217;elle est sp\u00e9cifiquement r\u00e9gul\u00e9e \u00e0 la hausse dans les m\u00e9tastases c\u00e9r\u00e9brales. De plus, cette m\u00e9thode a d\u00e9j\u00e0 permis de r\u00e9duire la croissance macrom\u00e9tastatique et la r\u00e9gression des microm\u00e9tastases. Pour cette \u00e9tude, une microscopie biphotonique longitudinale in vivo a \u00e9t\u00e9 r\u00e9alis\u00e9e \u00e0 travers une fen\u00eatre cr\u00e2nienne chronique. En outre, une microscopie mol\u00e9culaire in vivo, une microscopie de fluorescence et une mesure de bioluminescence chez la souris ont \u00e9t\u00e9 utilis\u00e9es. Les observations ont montr\u00e9 une activation stable et durable, n\u00e9cessaire \u00e0 l&#8217;extravasation et \u00e0 la formation de m\u00e9tastases c\u00e9r\u00e9brales, m\u00eame en cas d&#8217;arr\u00eat intravasculaire pr\u00e9coce des cellules tumorales dans les vaisseaux c\u00e9r\u00e9braux. Apr\u00e8s colonisation du cerveau, les cellules tumorales ont montr\u00e9 une activation stable de la voie de signalisation, qui a \u00e9t\u00e9 r\u00e9duite lors de l&#8217;administration de GNE-317, un inhibiteur double de PI3k\/Akt. L&#8217;administration pr\u00e9ventive d&#8217;une faible dose de GNE-317 a en effet entra\u00een\u00e9 une r\u00e9duction de la charge des m\u00e9tastases c\u00e9r\u00e9brales et une augmentation de la survie globale par rapport \u00e0 un traitement des m\u00e9tastases \u00e9tablies avec une dose plus \u00e9lev\u00e9e ou un groupe t\u00e9moin. L&#8217;\u00e9tude a permis de d\u00e9montrer, \u00e0 l&#8217;aide de la microscopie mol\u00e9culaire, le r\u00f4le important de la voie de signalisation PAM dans le d\u00e9veloppement des m\u00e9tastases c\u00e9r\u00e9brales et de prouver que l&#8217;inhibition de PI3k\/mTOR est une mesure pr\u00e9ventive.
