Outre le diagnostic et la surveillance, ils semblent \u00e9galement pouvoir donner une indication pr\u00e9coce des effets secondaires cardiovasculaires en cas de maladie maligne.<\/strong><\/p>\n<p> <!--more--> <\/p>\n<p>La meilleure compr\u00e9hension et les possibilit\u00e9s de traitement plus efficaces des maladies malignes qui en d\u00e9coulent se traduisent par un nombre croissant de survivants \u00e0 long terme. Il est donc de plus en plus important de conna\u00eetre rapidement les \u00e9ventuels effets secondaires cardiovasculaires associ\u00e9s au traitement. Une m\u00e9ta-analyse a examin\u00e9 plus en d\u00e9tail les biomarqueurs cardiaques troponine (cTN) et peptide natriur\u00e9tique c\u00e9r\u00e9bral (BNP\/NT-proBNP) en termes d&#8217;efficacit\u00e9 pour la d\u00e9tection de la cardiotoxicit\u00e9 associ\u00e9e au traitement dans les traitements anticanc\u00e9reux.<\/p>\n<p>Au total, 5 664 patients issus de 57 \u00e9tudes ont \u00e9t\u00e9 analys\u00e9s. Le crit\u00e8re d&#8217;\u00e9valuation principal a \u00e9t\u00e9 d\u00e9fini comme une r\u00e9duction de la fraction d&#8217;\u00e9jection du ventricule gauche (FEVG) au-dessus d&#8217;un seuil fix\u00e9. Il s&#8217;est av\u00e9r\u00e9 que la chimioth\u00e9rapie conventionnelle et le traitement par inhibiteur HER2 \u00e9taient associ\u00e9s \u00e0 une augmentation de la troponine (OR : 14,2 ; IC \u00e0 95% : 7,5-27,0). Sur le plan pronostique, cela indiquait le d\u00e9veloppement d&#8217;un dysfonctionnement du ventricule gauche. L&#8217;effet le plus fort a \u00e9t\u00e9 d\u00e9montr\u00e9 chez les patients subissant une chimioth\u00e9rapie \u00e0 haute dose. Avec une valeur pr\u00e9dictive n\u00e9gative de 94% pour la pr\u00e9diction de la cardiotoxicit\u00e9, la troponine a \u00e9t\u00e9 identifi\u00e9e comme un marqueur avantageux pour l&#8217;examen des patients \u00e0 risque. Toutefois, cette conclusion ne s&#8217;applique pas au BNP\/NT-proBNP. Une valeur pathologiquement \u00e9lev\u00e9e n&#8217;\u00e9tait pas significativement associ\u00e9e \u00e0 l&#8217;incidence de la dysfonction ventriculaire gauche.<\/p>\n<p><em>Source : Michel L, et al : Clin Res Cardiol 2019 ; 108, Suppl 1.
