Quels sont les avantages cliniques des inhibiteurs de PCSK-9 pour la r\u00e9duction du cholest\u00e9rol ?<\/strong><\/p>\n<p> <!--more--> <\/p>\n<p>\u00c0 la naissance, la concentration de cholest\u00e9rol LDL est inf\u00e9rieure \u00e0 50 mg\/dl et augmente ensuite jusqu&#8217;\u00e0 l&#8217;\u00e2ge adulte. Des valeurs de 125 mg\/dl ne sont alors pas rares. Toutefois, selon des informations r\u00e9centes, un taux de cholest\u00e9rol LDL tr\u00e8s bas r\u00e9duit d&#8217;un tiers le risque d&#8217;infarctus et d&#8217;accident vasculaire c\u00e9r\u00e9bral. Il est possible de r\u00e9duire les taux en inhibant la HMG-CoA r\u00e9ductase par des statines, l&#8217;absorption du cholest\u00e9rol par l&#8217;\u00e9z\u00e9timibe ou en augmentant la d\u00e9gradation des LDL \u00e0 l&#8217;aide d&#8217;inhibiteurs de PCSK-9, comme l&#8217;explique le professeur Franz. R. Eberli, Zurich, a montr\u00e9. Ces derniers ont d\u00e9j\u00e0 prouv\u00e9 leur efficacit\u00e9 dans plusieurs \u00e9tudes. &#8220;Les inhibiteurs de PCSK-9 r\u00e9duisent le cholest\u00e9rol LDL de 60%&#8221;, a rapport\u00e9 l&#8217;expert. Ils marquent \u00e9galement des points en termes de s\u00e9curit\u00e9 : Aucun signe de troubles musculaires ou neurocognitifs n&#8217;a \u00e9t\u00e9 d\u00e9tect\u00e9. Les anticorps sont tr\u00e8s rares et les r\u00e9actions allergiques ne sont que sporadiques.<\/p>\n<p>Mais qu&#8217;en est-il de l&#8217;utilit\u00e9 clinique de ce nouveau r\u00e9gime th\u00e9rapeutique ? Les grandes \u00e9tudes de crit\u00e8re d&#8217;\u00e9valuation FOURIER et ODYSSEY ont surtout d\u00e9montr\u00e9 une r\u00e9duction du taux d&#8217;infarctus du myocarde (1,9% et 1,0% respectivement) et une r\u00e9duction des \u00e9v\u00e9nements cardiovasculaires (2% et 1,6% respectivement).
