La substitution a de nombreux effets b\u00e9n\u00e9fiques et semble \u00e9galement avoir un impact positif sur la pr\u00e9vention des exacerbations dans la BPCO.<\/strong><\/p>\n<p> <!--more--> <\/p>\n<p>La revue syst\u00e9matique et la m\u00e9ta-analyse [1] ont regroup\u00e9 les patients de quatre essais contr\u00f4l\u00e9s randomis\u00e9s (ECR) (n=560) qui avaient re\u00e7u de la vitamine D3 par voie orale contre un placebo. Les patients ont \u00e9t\u00e9 \u00e9valu\u00e9s en termes de taux d&#8217;exacerbations mod\u00e9r\u00e9es \u00e0 s\u00e9v\u00e8res.<\/p>\n<p>Ce r\u00e9sultat a \u00e9t\u00e9 pr\u00e9sent\u00e9 par le professeur Claus Vogelmeier, de la clinique de m\u00e9decine interne, sp\u00e9cialis\u00e9e en pneumologie, de l&#8217;h\u00f4pital universitaire de Giessen et Marburg, lors du Pneumo-Update \u00e0 Mayence (Allemagne) : Lors de l&#8217;\u00e9valuation de l&#8217;ensemble de la population, le taux d&#8217;exacerbation mod\u00e9r\u00e9e\/grave de la BPCO dans le groupe d&#8217;intervention \u00e9tait de 0,94. Ainsi, aucun effet pertinent de la substitution n&#8217;a pu \u00eatre mis en \u00e9vidence. Cependant, lorsque les patients ont \u00e9t\u00e9 ventil\u00e9s en fonction de leur taux de vitamine D, un signal clair a \u00e9t\u00e9 observ\u00e9 dans le sous-groupe pr\u00e9sentant un faible taux de vitamine D \u00e0 la base : des effets protecteurs ont \u00e9t\u00e9 d\u00e9tect\u00e9s chez les participants pr\u00e9sentant un taux de vitamine D mesur\u00e9 au d\u00e9part &lt;25&nbsp;nmol\/l (RR ajust\u00e9 de 0,55). En revanche, lorsque les taux de vitamine D \u00e9taient sup\u00e9rieurs ou \u00e9gaux \u00e0 25&nbsp;nmol\/l, cet effet n&#8217;a pas \u00e9t\u00e9 observ\u00e9 (RR ajust\u00e9 de 1,04) <strong>(Fig. 1).<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p><img fetchpriority=\"high\" decoding=\"async\" class=\" size-full wp-image-13060\" alt=\"\" src=\"https:\/\/medizinonline.com\/wp-content\/uploads\/2019\/02\/abb1_pa3_s36.png\" style=\"height:307px; width:600px\" width=\"1100\" height=\"563\"><\/p>\n<p>&nbsp;<\/p>\n<p>En pr\u00e9sence d&#8217;un taux r\u00e9duit de vitamine D, une substitution peut donc \u00e9ventuellement avoir des effets b\u00e9n\u00e9fiques significatifs sur le taux d&#8217;exacerbation. Il convient donc de se demander \u00e0 l&#8217;avenir si l&#8217;administration de ce m\u00e9dicament ne pourrait pas constituer une mesure pr\u00e9ventive chez les patients souffrant d&#8217;exacerbations r\u00e9currentes.<\/p>\n<h2 id=\"conclusion\">Conclusion<\/h2>\n<ul>\n<li>Une substitution en vitamine D doit \u00eatre envisag\u00e9e chez les patients pr\u00e9sentant des exacerbations r\u00e9currentes et des taux sanguins faibles.<\/li>\n<li>La th\u00e9rapie est peu co\u00fbteuse et s\u00fbre.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>Litt\u00e9rature :<\/p>\n<ol>\n<li>Jolliffe DA, et al : Vitamine D pour pr\u00e9venir les exacerbations de la BPCO.
