A patient on SGLT2 inhibitor therapy developed euglycemic diabetic ketoacidosis, which ultimately led to a diagnosis of latent autoimmune diabetes in adults (LADA). The misdiagnosis of type 2 diabetes delayed the necessary insulin therapy and increased the risk of ketoacidosis through the use of SGLT2 inhibitors, as these drugs can promote ketogenesis, especially in the presence of insulin deficiency. The case underlines the importance of early differentiation of forms of diabetes using C-peptide and antibody tests.
