Onychomycosis diagnosis by histology and/or culture is mandatory before any systemic therapy. Dermatophytes are among the most common fungal pathogens. Systemic therapy – preferably with Terbinafine – should always be combined with topical treatment. In the case of nail infestation of less than 50% without matrix infestation, topical therapy is sufficient.
