With the guideline published on December 1, 2025, the World Health Organization has, for the first time, issued a globally applicable recommendation on the pharmacological management of obesity in adults. It was released simultaneously with an accompanying article in the Journal of the American Medical Association. This is more than just a technical update: The WHO thereby explicitly recognizes obesity as a chronic, recurrent disease that—similar to hypertension or diabetes—requires long-term, structured treatment. For general practice, diabetes care, and cardiology practices, the question is therefore less whether GLP-1-based therapies have a place, and more about for whom, under what circumstances, and with what responsibilities they should be used.
