The year 2025 has brought several landmark documents to reproductive medicine: the first WHO guideline on the prevention, diagnosis and treatment of infertility, the major update of the ESHRE guideline on ovarian stimulation with 46 new recommendations, the updated ESHRE/ALPHA-Istanbul consensus on oocyte and embryo evaluation and new consensus documents on IVF add-ons. In parallel, data on pre-implantation genetic testing (PGT-A), recurrent implantation failure research and the growing importance of sperm DNA fragmentation raise fundamental questions for routine clinical practice.
